TERMS OF AGREEMENT
Please read the following carefully before continuing your registration.
1. The client/patient acknowledges that Gates for Weight Management is a nutrition clinic owned and run by Prof Dr Mohamed Aboulghate. This entire website is considered an online extension to his clinic designed to provide the same service online and is run under his authority and supervision.
2. The client/patient hereby certifies that he/she is willing to deal with Gates for Weight Management clinic for nutritional counselling in the form of setting a nutrition plan aiming pimarily for weight loss.
3. The client/patient acknowledges that he understands that the service(s) provided by Gates for Weight Management clinic are strictly limited to the field of nutrition and dieting, and should not be expected to involve any other branch of medicine or any service not related to this specific field.
4. The client/patient gives full right to Gates for Weight Management clinic to use and store his information and medical data on the clinic system, but NOT to publish or disclose any form of data concerning the patient except at his/her permission. In return, all registered users are responsible for the security of their accounts and their password. In any case of a stolen password or illegal access or use of any user’s account, the clinic will not be held responsible for information disclosure and must be informed immediately.
5. The client/patient acknowledges that )s)he is responsible for providing accurate and up-to-date information regarding his personal and medical information. Any adverse events resulting from providing incomplete, false or inaccurate data provided by the client/patient throughout any phase or form will not be the responsibility of the clinic in any way. This applies also to any adverse events that happe as a result of not following the instructions provided by the clinic team. The patient is also expected to update his/her data (especially medical data) continuously and accurately through his/her profile.
6. The client acknowledges that certain investigations (Medical lab tests) might be required by the clinic to be done by the client/ patient and will be specified to the client/patient if needed. Although the account of the patient shall not be closed if the investigations are not uploaded to the clinic’s system, the patient will be held responsible for not uploading or delaying them.
7. The client/patient agrees to the fact that all forms of data on the website including the website pages and design, clinic’s logo, pictures and other multimedia or content, diets and clinic forms or technical information, is the sole property of Gates for Weight Management clinic and any form of reproduction, illegal/unauthorized use or distribution of such data will be considered an act of copyrights breaching and lead to judicial prosecution.
8. Gates for Weight Management clinic reserves the right to deny access to the follow up form to any client/ patient, or to block/close down their account in the case of delayed payment. The clinic also reserves this right for any client/patient who performs any form of illegal or unacceptable behavior or uses any inappropriate language while using the online system. In such case the client/patient’s account shall be closed and he/she will be refunded the remaining payment deposit immediately.
9. Although Gates for Weight Management could provide the online service to people from all age groups, children under 16 years of age are strongly encouraged to be accompanied by a parent at least during the initial registration process. The clinic will not be responsible for any children who use this website in any form unsupervised.
10. Gates for weight Management clinic reserves the right to modify or add to all or any part of this agreement without any prior notice. This condition also applies to changes in prices of the clinic or online services.
11. The client/patient agrees to the following terms of payment:

-Payment shall be made in advance in the form of a deposit payment which could be paid through a number of payment methods, both online and offline.

-The fees for our services are 700 Egyptian pounds which entitles the patient to receive 4 weekly consultations over a period of one month.

-Online payments on our website are through the Arab African International Bank gate and are subject to its online payment security measures. In cases of a breach of these measures by a third party, the clinic will not be responsible for any financial losses or other damages incurred by the customer.

-In case a client/patient has paid a certain deposit and does not wish to continue with the clinic, he/she can refund all the remaining sum of money according to the number of follow up visits/sessions (s)he has received.
